General Information for the Survey:

• The term “adoption provider” refers to an adoption agency, attorney or other adoption professional that assisted you with placing your child for adoption.
• Reviews are accepted by birth parents only!
• Birth parents should carefully review the list of agency names.  Many agencies have very similar names.  We want to be as accurate as possible when posting reviews.
• Agencies and attorneys are permitted to leave a response regarding a posted review, but you must log in and identify yourself as a provider.
• If the provider you are seeking to review is not listed, please submit their name, city, state, and phone if possible in the comments section of the review.  Without this information we will be unable to post your review.
